Initial Consultation

The brief initial consultation is an opportunity for us to get to know each other. This consultation takes place via phone. You will be asked to complete a short Intake Form as well prior our conversation. At this time, I am only seeing patients who live in Illinois, Virginia, Florida, Minnesota or South Dakota.
Comprehensive Evaluation
Time: up to 60 min
Method: Telehealth
Fee: $450
This is an in-depth evaluation of biological, psychological and social components of your subjective life experience. I utilize all three entities to help us understand how and why you are experiencing certain symptoms and difficulties. You will be asked to complete a Patient History Form prior to the appointment. Note: Sliding scale fees may available for those in serious financial need.
60 min Follow Up Appointment
Time: 55-60 min
Method: Telehealth
Fee: $300
Follow-up appointments combine nutritional, lifestyle, psychopharmacological and psychotherapeutic interventions depending on an individual's treatment plan. We will review your medication treatment plan as well. We will check in at each visit to see how the treatment is progressing. Note: Sliding scale fees may be available for those in serious financial need.
30 min Follow Up Appointment
Time: 25-30 min
Method: Telehealth Fee: $150
Brief follow-up appointments allow for us to briefly touch base, answer any important questions, and make some adjustments to treatment plan including medication. Note: Sliding scale fees may be available for those in serious financial need.
Group Therapy
Time: up to 60 min
Method: Telehealth
Fee: $600 for a 10wk session
Group therapy allows for an amazing combination of individual and group support. You learn from you own subjective experience, and also from the healing observed by others around you. We investigate interpersonal dynamics of you and the group--dynamics that often contribute to psycho-social troubles in everyday life.
Telephone Consultations
Time: 10 min or more
Method: Phone
Fee: Pro-rated at $300/hr ($5/min)
Although I would prefer to see you in person or via the tele-health platform, I understand the importance of brief telephone exchanges periodically in between appointments. Calls lasting longer than 10min may be considered a telephone consultation and may be subject to appropriate fees.
Miscellaneous Paperwork (Letters, Forms, etc.)
Time: 5 min or more
Method: Phone/Email
Fee: Pro-rated at $300/hr ($5/min)
I am happy to help complete any letters or forms necessary for legal, insurance or other reasons. I will work with you to decide what forms/letter I would be best to help you with and direct you to other professionals if necessary. Paperwork filled out outside of a scheduled appointment may be subject to appropriate fees.
Collaboration of Care
Time: 15 min or more
Method: Phone/Email
Fee: Pro-rated at $300/hr ($5/min)
It is very important for your treatment team including your primary care provider, therapist, other physicians and specialists stay in close communication. I am happy to collaborate with all members of your team if you wish me to do so. Time intensive collaboration conducted outside of the scheduled appointment time may be subject to hourly fees for service ($300/hr).
